The final step to finalize your unclaimed property claim is to provide proof of your identification. We recommend that you send a copy of your government issued ID card (Driver’s license, passport, etc…) along with your contract agreement, but with all the scam in our society today, we understand that you might not feel comfortable doing so.

The Notarized Sworn Statement form can be used as an alternative way to prove your identity. Please mail the original notarized copy back to us.

If the claim involves a deceased owner, and the total asset is less than $10,000.00, please fill out the Estate Affidavit and mail the form back along with the Limited Power of Attorney (LPOA) and all other relevant documents needed for your claim.
The Estate Affidavit verifies your relationship with the owner and will validate your claim.
